Physician Assistant (PA) – Nursing Home Primary Care
Location: Cortland, NY
Schedule: Monday–Friday | Flexible Schedule | Option for 4-Day Workweek
Compensation:
$130,000–$150,000 Base Salary + Quarterly RVU Bonuses
MedStaff Group is recruiting on behalf of a growing medical group seeking a Physician Assistant to provide primary care services within skilled nursing facilities throughout the Cortland, NY area.
This position offers autonomy, schedule flexibility, and the opportunity to build long-term relationships with geriatric patients while working alongside experienced facility and clinical teams.

Responsibilities
Provide primary care services to nursing home residents
Conduct routine visits, follow-ups, and chronic disease management
Collaborate with nursing staff and facility leadership
Participate in care management programs including CCM and RPM
Maintain accurate documentation in SigmaCare, PCC, or similar EMR systems
Requirements
Active New York Physician Assistant License
Active Medicare and Medicaid Provider ID
Experience in primary care, geriatrics, or long-term care preferred
Skilled Nursing Facility experience is a plus
Strong clinical judgment and communication skills
