(Full time) Open to PT as well.

What You’ll Do
Round on patients daily, building real continuity of care
Diagnose and manage a wide range of acute and chronic conditions
Work hand-in-hand with interdisciplinary care teams and facility leadership
Chart smarter, not harder — with AI-integrated EHR documentation built for speed
What You Bring
Active or eligible OR PA license
DEA and national certification (or ability to obtain)
SNF experience is a plus, but not required — we love developing new grads
