Case logs last 24 months
Must have NH license and DEA at the time of presentation
Must have clean malpractice/background
Must be Board Certified in Emergency Medicine
Must send self-query NPDB and EPA with presentation
Must submit COVID vaccination card
ACLS needed if requesting sedation privileges
