nAnthony L. Jordan Health Corporation is seeking a Board Eligible or Board Certified Family Medicine Physician to join one of the nation's oldest Federally Qualified Health Centers serving the Rochester community for more than 120 years.

nRequirements

nResponsibilities

nnnProvide comprehensive outpatient primary care to pediatric, adult, and geriatric patients.

n

nnPerform preventive care, acute care visits, chronic disease management, and health screenings.

n

nnDevelop and maintain continuity of care for an assigned patient panel.

n

nnCollaborate with nurses, behavioral health providers, pharmacists, care managers, and other interdisciplinary team members.

n

nnRefer patients to specialty care and community resources as appropriate.

n

nnUtilize evidence-based medicine and participate in quality improvement initiatives.

n

nnServe as a preceptor for residents and students as assigned.

n

nnMaintain accurate and timely documentation in the electronic health record.

n

nnParticipate in departmental meetings and organizational initiatives.

n

n nEducation and Experience Required:

n· Possession of a license to practice as a medical doctor in New York State.

nLicenses and Certifications:

n· Board eligibility or Board certification.

n· Must obtain credit of CME as required.

n· Must meet privileging and credentialing requirements for Physicians.

n· Infection Control certificate.

n· BLS through the American Heart Association.

nSpecial Skills, Knowledge Required:

n· Ability to administer impartial treatment to a wide cross segment of the socio-economic spectrum.

n· Ability to work under conditions of stress.

n· Skill in interpersonal relationships.

n· Proficiency in MS Office Applications (Word, Access, Excel, Power Point, Outlook) and eClinicalWorks or other electronic health record.
