Hours: Monday - Friday 8 am - 5 pm

Sign-On Bonus
Compensation: - Base salary plus bonuses with earning potential up to $300,000- Provider incentive bonus is performance based not RBU or fee for service

Qualifications:
- 1 year of experience required- FM, IM or Geriatric trained- Experience working with senior populations or in Value-Based Care settings.
