Board Certified in Family Medicine or Internal Medicine. - REQUIRED
Active DEA certification. - REQUIRED
Minimum of one (1) year of physician experience required. - REQUIRED
Active state medical license required at time of submission. - REQUIRED
At least one professional reference within 24 months - REQUIRED
Does your candidate require visa sponsorship (NOT ACCEPTING VISA CANDIDATES) - REQUIRED
Clean malpractice and license history HIGHLY PREFERRED
