Board Certification: Required
License: Active VA license required
Certifications: BLS, PALS, ACLS
Provider Must be Local: Only Mileage is Reimbursed - note at the time of name clear

Average Number of Patients Per Day: 18
Number of Exam Rooms: 16
Services Available On-Site: Laboratory and Radiology
Documentation System/EMR: EPIC
Emergency Supplies Available: Defibrillator
Required General Procedures: Evaluation and Mgmt. of Acute Volume / BP Issues
Required Medicine Procedures: Adult Outpatient and Without ICU/CCU
Required Pediatric Procedures: General Outpatient
Required Gynecology Procedures: Pelvic Exam / Pap Smear
