Job Description\ N Seeking BE/BC Family Medicine w/OB Physician to join a thriving rural practice including six Family Medicine physicians and four Advanced Practice Providers providing a full spectrum of care in Central Minnesota.\ N

Call approximately 1:6 weekdays with physician staffing Level IV Emergency Department during day and providing back-up call to APP overnights.

\ N Weekend call with primary hospitalist duties including admissions and rounding. Locums ED coverage means more time at home! Additional ED shifts available for those interested.

\ N Opportunity to grow obstetrical practice while providing OB back-up call to partners.

\ N Salary Range $250,000 -$300,000

The Clinic: \ N Dedicated primary clinic nurse (CMA, LPN or RN) with support of float nursing

\ N Co-located workstationsfor provider/nursing with an additional office

\ N Opportunity for voice-recognition software (Dragon) or scribe

\ N Team support including RN nurse lead visits, diabetic education, health care home for complex patient care, legal aide, health care navigator for uninsured/underinsured

\ N Community health workerand community paramedic for patient engagement and home visits

\ N Prescription support programs for uninsured/underinsured

\ N In-room schedulers to promote continuity (follow-up appointments made for patients before they leave the room)

\ N Mental Health support including Integrated Behavioral Health with often same-day video consultation and Psychiatry chart review for medication management

\ N E-consults for specialty care to allow for PCP management without the wait for a formal consult
