* Suburban Charlotte, North Carolina location
* Outpatient only physician
* Value-based care model
* Medicare/Medicaid Advantage patients
* All patients are 65+
* Monday-Friday office only schedule
* Phone call shared with all providers
* Minimal phone call once every few months
* 12-15patients per day
* Full supports staff including NP-MA-RN
* On-site labs and radiology
Compensation
* Total compensation $300,000 and above year one
* Attainable 20% salary bonus plan
* $60,000 Sign-on bonus
* Relocation allowance
