Job Description- Board Certified Nurse Practitioner or Physician Assistant with experience\ N
- Must speak Spanish
\ N- Make an impact on seeing the underserved population\ N
- Full-time permanentPrimary Care Outpatient Only position\ N
- Monday " Friday No nights, no weekends\ N
- Small patient panels for consistency and focus (2-15 patients per day)\ N
-Value-based care model\ N
-Provide personalizedprimary care for older adults on Medicare, with the goal of keeping patients healthy and living life to the fullest\ N
- Administer Annual wellness visits and health risk assessments, which require aholistic view of health and afocus on thoughtful, accurate, and specific documentation.\ N
- Population health leadership, in coordination with the Care Team (e.G., making sure all eligible females get their evidence-based breast cancer screening every 2years)\ N
- This role reports to the Center Medical Director and works closely with operational leadership.\ N
- Understands HCC (Hierarchical Condition Categories) documentation, ICD-10 (International Classification of Diseases-10) Coding, and Health Risk Assessments (HRAs).\ N
