Job description
Position OverviewWe are seeking a dedicated Physician to provide comprehensive medical coverage across Skilled Nursing Facilities (SNF) and Assisted Living Facilities (ALF) in the Long Island market. This role offers the opportunity to lead clinical oversight, manage medically complex patients, and reduce avoidable hospital transfers through proactive, person-centered care.Coverage Areas:Nassau County, Suffolk County, and select locations in Queens (depending on practice site)Key ResponsibilitiesSkilled Nursing Facility (SNF) / Post-Acute CareProvide dedicated Medical Directorship with emphasis on quality and complianceEnsure 7-day medical coverage with on-site and on-call availabilityOvernight coverage handled by in-house providers for continuity and responsivenessReduce rehospitalization rates below state and national averagesStreamline medication regimens to reduce polypharmacy and pharmacy costsEnsure appropriate and compliant psychotropic useManage medically complex and high-risk patients with expertise and compassionImplement proactive fall prevention and quality improvement programsAssisted Living Facility (ALF) Medical ServicesProvide ongoing medical directorship and clinical oversight for residentsConduct regular on-site visits by physicians and advanced practice providers (APPs)Maintain 24/7 availability for urgent and after-hours medical needsFocus on chronic disease management and preventive careEnsure appropriate psychotropic use and minimize unnecessary medicationsCollaborate closely with staff and families for coordinated, person-centered careReduce avoidable hospital transfers through proactive interventionAPP CollaborationAPPs see patients and provide fill-in coverage when neededParticipate in rounds and follow-back care as part of the teamCompensation & BenefitsBase Salary:$275,325 + productivity bonusFull Benefits:Medical, Dental, Life InsuranceDEA/License Reimbursement3 Weeks PTOQualificationsMD or DO from an accredited medical schoolBoard Certification (or eligible) in Internal Medicine, Family Medicine, or Geriatric MedicineActive, unrestricted New York State medical licenseActive DEA licenseUS work authorizationExperience in SNF/ALF settings preferredStrong commitment to person-centered care for medically complex, high-risk patientsWhy Join Us?Lead clinical quality and compliance across multiple SNF and ALF sitesWork with a collaborative team of physicians and APPsManage complex cases with expertise and compassionProactive fall prevention and quality improvement programsCompetitive compensation with productivity bonus opportunityFull benefits package including license/DEA reimbursementJoin a team dedicated to reducing rehospitalizations, minimizing polypharmacy, and delivering coordinated, person-centered care to residents across Long Island
