Employment Type: Full-Time
Salary Range: $225,000 – $300,000 (based on experience)
\ NA growing post-acute careorganization is seeking a compassionate, mission-driven Physician to join its expanding clinical team. This role focuses on providing high-quality primary care to residents in long-term care settings, supporting a healthier, more dignified aging experience.
About the Role \ NIn this position, you will provide comprehensive primary care to adults and seniors residing in long-term care communities. You’ll work in a supportive, physician-focused environment that prioritizes clinical quality, collaboration, and work-life balance.
\ NThis is an ideal role forexperienced physicians or recent graduates who want to practice meaningful medicine with predictable daytime hours and strong organizational support.
Responsibilities \ N Conduct admissions, evaluations, routine assessments, and treatment plans
\ N Manage chronic and acute medical conditions in the LTC environment
\ N Collaborate closely with interdisciplinary care teams including NPs, nurses, therapists, and facility partners
\ N Communicate with families and caregivers to support continuity of care
\ N Document encounters accurately using a provided mobile device
\ N Provide compassionate, patient-centered care with a focus on dignity and quality of life
\ N Schedule \ N Monday–Friday, daytime hours only
\ N Minimal on-call responsibilities
\ N No nights, weekends, or holiday rounding
\ N Predictable schedule that supports work-life balance

Qualifications \ N Board Eligible or Board Certified Internal Medicine or Family Medicine Physician
\ N Interest in geriatrics, long-term care, or post-acute medicine
\ N Excellent teamwork and communication skills
