Primary Care Physician Holiday, FL Compensation Total earning potential: Up to $312,000 Annual bonus up to 20% of base salary Bonus based on: Patient satisfaction Living company values HEDIS compliance Reducing hospital sick days About the Opportunity Innovative, value-based primary care organization Outpatient-only practice Focus on senior patients with complex chronic conditions Neighborhood clinics designed to provide accessible, community-centered care Wellness-focused environment offering: Uber/Lyft transportation for patients Exercise, art, and wellness programs at no cost Practice Details Outpatient Primary Care Value-Based Care model Patient population: Seniors with 35 chronic conditions (heart disease, diabetes, hypertension, etc.) 1518 patients per day Maximum panel size of 650 patients Shared call schedule based on market provider count (example: 1:32 weeks if 32 physicians) Dedicated support team for each physician, including: Medical Assistant Scheduler Referral Specialist Leadership support Requirements MD or DO Board Certified or Board Eligible in Family Medicine or Internal Medicine Geriatrics fellowship preferred, but not required Current, unrestricted medical license Current DEA registration Contact directly: Bhavini Scianablo
