\ N Outpatient only physician
\ N Value-based care model
\ N Medicare/Medicaid Advantage patients
\ N All patients are 65+
\ N Monday-Friday office only schedule
\ N Phone call shared with all providers
\ N Minimal phone call onceevery few months
\ N 12-15patients per day
\ N Full supports staff including NP-MA-RN
\ N On-site labs and radiology
\ N Compensation \ N Total compensation $300,000 and above year one
\ N Attainable 20% salary bonus plan
\ N $60,000 Sign-onbonus
\ N Relocation allowance
\ N Matching 401k retirement plan
