Job description
HarmonyCares seeks a Physician Leader in Youngstown, OH to provide home-based primary care to medically complex, home-limited adults. In this role, you will oversee an interdisciplinary team delivering in-home visits, telehealth, and palliative-focused care. You’ll guide care plans, reduce avoidable hospitalizations, and mentor clinicians while modeling our patient-first, compassionate culture. This position offers a $50K retention bonus, leadership growth, and the opportunity to improve access and outcomes for vulnerable patients in a mission-driven setting.
