Job description
HarmonyCares is seeking a Physician Leader for Long Island and Queens, NY to direct home-based primary and palliative care for medically complex, home-limited patients. In this role, you will provide direct in-home clinical care, guide an interdisciplinary team, and champion a patient-first, compassionate culture. You will coordinate telehealth and in-person visits, develop individualized care plans, and work to reduce avoidable hospitalizations. This position offers the opportunity to drive clinical quality, mentor clinicians, and help shape a mission-driven, rapidly evolving care model.
