In this role, you will provide direct in-home and telehealth primary care to medically complex, home-limited adults while leading an interdisciplinary team of NPs, nurses, social workers, and care coordinators. You will oversee clinical quality, support value-based care initiatives, and reduce avoidable hospitalizations through proactive care planning.
