Responsibilities - Provide comprehensive primary care, acute care, and chronic disease management. - Deliver preventive care and health screenings tailored to individual patient needs. - Collaborate within a multidisciplinary care team to address overall medical and social needs. - Maintain accurate, timely documentation in the electronic health record (EHR). - Participate in quality improvement and population health initiatives. - Ensure compliance with HRSA, HIPAA, and state and federal regulations.
