Job DescriptionType of practice:\ N

o Multi-specialty (23 different specialties), outpatient, physician owned facility. Our main campus is in Fort Walton Beach and we also have satellite offices in Destin, Niceville and Navarre.\ N

- Type of employment:\ N

o First 2 years, associate contract position. Immediately following the 2-year Associate contract, there is the option to be voted in as a Shareholder.\ N

- Number of physicians:\ N

o Neurology: 3 doctors, 1 extender\ N

o Total in clinic: 46 doctors, 24 extenders (PA or ARNP) - Office hours: o Monday-Friday, 8-5pm o 36-hours of Direct Patient Contact\ N

Requirements:

o US Resident or Green Card\ N

o Board Certified or Board Eligible (will be required to take the test after completing residency/fellowship)\ N

o 1:4 call from 7 am – 5 pm / telephone neurology overnight\ N

o Telephone for office when on call\ N

- Average number of patients per day: 18\ N

- Inpatient work:\ N

o Contingent on casespresented when on call and the treatment/testing requirements for that case (EEG’s, EMG’s, Advance Ancillary Orders, Lumbar Punctures)\ N

o Hospital: Medical Center, located right across the street from Clinic\ N

o We are a Patient Centered Medical Home (PCMH) NCQA certified practice\ N

o In practice for over 70 years, strong presence in the community as the area’s largest physician owned, multi-specialty, outpatient clinic\ N

o All of our locations are within 10-20 miles from our main campus in Fort Walton Beach.\ N
