Job description
Work Dates Needed: 11/12-11/13 and 12/3-12/4
Preferred Schedule: 6am-4pm (all four days)
Worksite Setting: Outpatient - Occupational Med
Scope of Work:
(120) Physicals total basic, respiratory & hazmat
Health History Questionnaire (all)
OSHA Questionnaire (respiratory & hazmat)
Bloodwork: CBC & CMP, Lipid Panel
EKG's- only if recommended by provider
Fit Testing
PFT (for all)
UA dip
Vision (near, far, peripheral, color)
Audiogram
X-Rays (1v)
Licenses, Certifications, Requirements: KY license
