General MyMichigan Health Credentialing RequirementsAll Medical Schools must be on the California Approved List All Residencies & Fellowships must be ACGME ApprovedNo large malpractice issuesClean NPDBThree professional references that have worked with the provider within 18 monthsActive MI license (IMLC accepted), DEA, and Cs3Case/patient logs 24 Hematology patients within 12 monthsBoard Certified or Board Eligible within 5 yearsMyMichigan Medical Center MidlandHematology/Oncology Outpatient Clinic: Monday-Friday, 8-4:30Call Required about 2-5 calls/nightVolume of cases Approximately 15 visits/dayOutpatient visits consist of new patient referrals, established patients actively in treatment and follow-up appointmentsNo surgeries or procedures requiredBLS required24 patients with illness related to the practice of Hematology within 12 monthsName Clears to Haley Doede.
