Job description
Primary Care Attending Physician
i4 Search Group is supporting a search for a full-time Primary Care Physician to deliver on-site care at skilled nursing and long-term care facilities in the Danbury, CT region, alongside providing telehealth primary care services. This role combines direct clinical care with quality management initiatives to optimize senior care.
Key Responsibilities
In-Person & Virtual Care: Travel to provide on-site care at long-term care facilities; conduct virtual assessments and manage chronic/acute conditions via telehealth.
Care Coordination: Collaborate with interdisciplinary teams and maintain accurate medical records.
Quality Management: Support compliance audits, clinical evaluations, and regulatory adherence.
Qualifications
MD or DO from an accredited institution.
Extensive experience in long-term care (LTC) or skilled nursing facilities.
Strong leadership, communication, and electronic medical record (EMR) proficiency.
Knowledge of regulatory guidelines and quality improvement processes.
