Job DescriptionJob Summary:
\ NOne of our direct clientsis seeking a dedicated Physical Therapist (PT) to join their Home Health team in Kings County, CA/Colorado Springs, CO. The ideal candidate will provide high-quality, patient-centered care, helping individuals restore mobility, improve functional independence, and enhance overall quality of life in a home-based setting.
\ N Key Responsibilities:
\ N Conduct initial evaluations and ongoing assessments in patients’ homes
\ N Develop and implement individualized treatment plans based on physician orders
\ N Provide skilled therapy services including therapeutic exercises, gait training, balance training, and manual therapy
\ N Educate patients and caregivers on home exercise programs and safety techniques
\ N Monitor patient progress and adjust treatment plans as needed
\ N Complete accurate and timely documentation in compliance with regulatory standards
\ N Collaborate with physicians, nurses, and interdisciplinary team members
\ N Participate in case conferences and care coordination
\ N Qualifications:
\ N Active Physical Therapist (PT) license
\ N DPT or equivalent
\ N Experience(home health preferred)
\ N Strong clinical, communication, and organizational skills
\ N Ability towork independently in a field-based role
\ N Preferred Qualifications:
\ N Prior home healthexperience
\ N Experiencewith EMR systems (e.G., Homecare Homebase, WellSky)
