Summary: Ref #: 68048 Profession: Physician Specialty: Orthopedic Sports Surgery Job Type: Full-time Location: Davenport, IA Shift Schedule: Full time, Days Shift Type: Days Requirements: ABOS Board Certified or TRULY Board Eligible in Orthopedic Sports Medicine (must become certified within 5 years) REQUIRED no exceptions Valid IL & IA license or Active IMLC Willing to Obtain Licenses - REQUIRED Strong arthroscopic and open sports surgery skills REQUIRED , shoulder, knee, hip PREFERRED Hold current and unrestricted state and federal authority to prescribe and administer medications as necessary or appropriate to the field of practice - REQUIRED DEA - REQUIRED BLS Certification and maintain recertification - REQUIRED Clean malpractice and license history HIGHLY PREFERRED / PREFERRED Does the provider require visa support No J-1 or H-1B (Note in Additional Submission Details) REQUIRED Submit Self-Query NPDB pulled within 30 days from submission at time of presentation - REQUIRED BLS Board Certified Board Eligible Benefits: $100,000 recruitment incentive $15,000 relocation assistance 4 weeks PTO 1 week CME 6 paid holidays $6,000 CME allowance Health, Dental, Vision, Life, and Disability insurance Retirement: 401(k) with match plus 457(b) Professional liability coverage PSLF eligible
