Job description
The Legacy Senior Communities is seeking a Hospice Registered Nurse Case Manager in Plano, TX. This role coordinates care with the interdisciplinary team, patients, families, and referring agencies, ensuring delivery of comprehensive hospice services.
Required are graduation from an accredited nursing program, Texas RN license, and CPR certification. One year of RN experience in a clinical setting is required, hospice experience preferred, with on‑call availability as needed.
