Job description
Total Extended Care Services is seeking a Nurse Practitioner or Physician Assistant to join our PM&R team in the sub-acute rehab setting, working in person at skilled nursing facilities and partnering with a PM&R physician to improve patient outcomes.
In this role you will collaborate with therapy, nursing, and family teams to coordinate care, ensure timely charting, and support the goal of reducing readmissions while maintaining a patient-centered approach.
