Job description
Total Extended Care Services in Illinois is seeking a Nurse Practitioner or Physician Assistant to join our PM&R team in sub-acute rehab settings. You will partner with PM&R physicians and a multidisciplinary team to improve patient outcomes in skilled nursing facilities.
The role offers a flexible schedule with options for full-time or part-time work and no required on-call. Candidates must be licensed in IL and comfortable coordinating care across clinical teams.
