Telos Health Systems, Inc. is seeking an experienced Psychiatric Mental Health Nurse Practitioner (PMHNP) to provide geriatric care in Skilled Nursing/Assisted Living settings in Cookeville, TN.
The role features a daytime schedule on two nonconsecutive weekdays and a W2 compensation model. You will lead clinical care as the SNF-ist, collaborate with interdisciplinary teams, perform comprehensive assessments, manage medications, and develop patient management policies to improve residents’
