Job description
CommonSpirit Health is seeking a Population Health Coach RN to improve the clinically integrated network’s quality and value-based performance. You will manage referrals, monitor high-risk patients, and close gaps such as annual wellness visits and preventative screenings, using the Innovaccer platform for outreach.
You will collaborate with interdisciplinary teams, apply evidence-based metrics in practice, and maintain EMR and MS Office skills in a complex healthcare environment.
