Job description
Well Care Community Health, Inc. is seeking a Population Health Nurse to serve as a clinical lead for the chronic-disease hub.
You will translate EHR, registries, and RPM data into outreach and intervention while coordinating with Case Manager, CHW, providers, and pharmacy. You will develop RPM workflows, provide nursing assessment, educate patients, and ensure proactive, evidence-based care within licensed scope.
