Job description
The Population Health Pharmacist is a licensed pharmacist providing clinical pharmacy support to Medicare and Commercial providers, focusing on improving health outcomes and cost containment for Delaware residents through value-based care arrangements at Bayhealth.
Responsibilities include analyzing EMR data, performing MTM, and educating providers and members on formulary changes and health improvement plans with a goal of achieving high 5-STAR ratings.
