Job description
CommonSpirit Health is seeking a Population Health Coach in Nebraska City to collaborate with physicians and staff to improve quality and sustainability. You will manage chronic diseases through education, oversee referrals, interpret performance data to develop workflows, and implement Evidence-Based Metric guidelines for seamless transitions of care.
The role requires RN licensure (NE) with an associated degree, and experience or advanced education.
