Job description
Essen Medical Associates is seeking a Director of Population Health to lead all population health initiatives within a large Integrated Delivery System. The ideal candidate will have a Master's degree in a related field, at least 7 years of leadership experience, and a strong background in data analytics.
This role involves developing strategies to enhance care quality, overseeing compliance metrics like HEDIS, managing care coordination, and leading interdisciplinary teams toward better patient outcomes.
The salary range for this position is $145,000-$175,000.
