Job description
Hartford Healthcare is seeking a compassionate Revisit Nurse to provide follow-up care and assessments for patients after discharge across our network of hospitals and facilities. In this role, you will conduct in-person and virtual revisit appointments, monitor recovery progress, adjust care plans, and coordinate services with physicians and interdisciplinary teams. You will use our advanced EMR and clinical resources to deliver safe, high-quality care, reduce readmissions, and support patients and families through education and advocacy.
