Shift Details
PPV visit position. Minimum 10 visits per month. Must complete the weekday orientation process for training. Length may vary based on prior experience. Covers the greater Waterbury area.
Responsibilities
Provide skilled nursing care to clients in home and community-based settings, ensuring outcome-based, high-quality, cost-effective care.
Maintain client records using the agency’s standard documentation system.
Conduct follow-up visits to assess disease progress and evaluate treatment response.
Offer support to patients and families, applying teaching principles to promote and restore health and prevent disease and disability.
Coordinate care with providers and consult with physicians regarding client needs and care plans.
Communicate and confer with the Nurse Care Coordinator and all staff involved in the patient’s care.
Direct LPNs and Home Care Aides in carrying out the established plan of care, supervising and guiding them as needed.
Refer clients and families to community resources when required.
Attend and participate in in-service education programs.
Teach patients and reinforce client/caregiver education.
Collaborate and coordinate all client care services.
Minimum Requirements
Associate’s Degree or diploma program in nursing.
One year of nursing experience (or one year in a specialty).
Registered Nurse license to practice in the state of CT.
Preferred
Graduate of an NLN approved BSN program.
Medical/surgery or home care experience.
