Under the direction of the primary care operations team, the Primary Care Patient Care LPN works directly with Mercy Primary Care Providers to provide safe, timely, effective, efficient, equitable, patient-centered care in a systematic way. The Primary Care Patient Care LPN will partner with patients, providers, and other members of the clinical care team to drive quality outcomes for all patients. Hours/Schedule
Full-Time (40-hrs per week)
Days, Weekends
Minimum Requirements:
Education: Graduate of an accredited practical nursing program.
Licensure: Licensed as an LPN (Licensed Practical Nurse) in the state of practice.
Preferred Experience: 6 months clinical healthcare experience, preferably in a primary care office.

· Works well as a member of a team and willing to collaborate with all members of the care team

· Excellent organizational and interpersonal skills
· Flexible and positive attitude
· Attention to detail and accuracy
· Self-directed and able to perform tasks that are in scope of practice independently with minimal oversight
Physical Requirements:
Position requires individuals to push, pull, and/or lift 50 pounds on a regular basis.
Position requires prolonged standing and walking each shift.
Position requires the ability to grip, reach, bend, kneel, twist, and squat to perform duties.
