Job description
Join Hartford Healthcare’s Primary Care Behavioral Health team providing integrated behavioral health services within a leading not-for-profit system. In this role, you will collaborate with primary care providers to assess, diagnose, and treat patients with mild to moderate mental health and behavioral conditions. Responsibilities include brief interventions, care planning, crisis assessment, and coordinated referrals to specialty care. You’ll use evidence-based practices, electronic health records, and measurement-based care while contributing to an inclusive, team‑oriented culture focused on compassion, excellence, and continuous professional growth.
