Job description
Hartford Healthcare seeks a Primary Care Behavioral Health clinician to provide integrated mental health services within our primary care network. In this role, you will conduct psychosocial assessments, brief interventions, and care planning for diverse adult and pediatric patients. You’ll collaborate closely with primary care providers, monitor treatment progress, and coordinate referrals to specialty care.
