Job description
Hartford Healthcare seeks a Primary Care Behavioral Health clinician to integrate behavioral health services within our primary care network. In this role, you will provide brief assessments, evidence-based interventions, and care coordination for adult and pediatric patients with mental health and substance use concerns. You will collaborate closely with primary care providers, nurses, and social workers to design patient-centered care plans, support crisis management, and ensure appropriate referrals.
