Job description
Oak Street Health seeks a Behavioral Health Specialist (LCSW, LPC, or LMFT) to provide compassionate mental health care to adults on Medicare in underserved communities. In this role, you’ll conduct psychosocial assessments, deliver short-term and ongoing therapy, and create evidence-based treatment plans. You’ll collaborate closely with primary care providers, nurses, and care teams to address behavioral, social, and medical needs.
