Job description
Oak Street Health is seeking a Float Registered Nurse to support our primary care centers serving adults on Medicare. In this role, you’ll rotate between clinics, delivering patient-centered, preventative care, performing assessments, medication administration, care coordination, and chronic disease management. You’ll collaborate with physicians, nurse practitioners, and care teams to develop care plans, provide patient and family education, and support care transitions. The ideal candidate is adaptable, mission-driven, and passionate about improving health outcomes in underserved communities.
