Job description
Oak Street Health in Cleveland, OH is seeking a compassionate Nurse Practitioner to deliver high-quality, value-based primary care to adults on Medicare in underserved communities. You will perform comprehensive assessments, manage chronic conditions, and emphasize prevention and wellness in a patient-centered, team-based environment.
