Job description

SRCH is seeking a Family Nurse Practitioner or Physician Assistant to join the Patient Centered Medical Home team in Santa Rosa, CA. Under medical director supervision, you will provide comprehensive primary care, prevention, and health education, aligned with CCR regulations for FNP practice.

Responsibilities include patient exams, history taking, chronic disease management, and coordination with physicians to ensure quality care. Must hold CA FNP/PA license and have strong communication skills.
