Job description
Oak Street Health seeks a Part-Time Nurse Practitioner to deliver high-quality primary care to adults on Medicare in underserved communities. In this team-based model, you will perform comprehensive assessments, diagnose and manage acute and chronic conditions, and lead preventative care initiatives. You’ll collaborate closely with physicians, nurses, and care teams, use EHR tools to coordinate population health, and educate patients and families.
