Job description
Join a collaborative, patient-centered primary care team dedicated to improving health outcomes through meaningful relationships, preventive care, and whole-person wellness in Chula Vista, California. As a Primary Care Nurse Practitioner, you'll enjoy 10-15 patients per day, longer visits, and the opportunity to truly connect with your patients-without the pressure of high-volume medicine.

Compensation & Benefits $119,313 - $178,968 annually, plus annual incentive opportunities and sign-on bonus.

Requirements NP Board Certification with primary care, family medicine, or internal medicine background
Current, valid and unrestricted ARNP license and valid DEA number
English/Spanish bilingual proficiency is an essential job function
Minimum 3-5 years of primary care experience
Minimum 2+ years with case management emphasizing health prevention and population health management
Documentation of proof of a negative TB test in the last 12 months before start-date.
Team-based care experience
Experience working in EMR systems
Experience with Google Workspace
