Summary
Are you a Primary Care Nurse Practitioner looking for an opportunity to make a meaningful difference outside of a traditional medical office?
We are seeking an experienced Primary Care Nurse Practitioner to join an innovative Neighborhood Navigator Street Medicine Program serving individuals experiencing homelessness and substance use throughout Manhattan.
Approximately 80% of this position is community-based. You will work alongside an established team of Neighborhood Navigators to provide medical care directly in the communities where clients live and spend their time.
This is a unique, relationship-focused position. You will not be expected to work in a high-volume clinic or meet a traditional daily patient quota.
Instead, you will have the time to build trust, address immediate medical needs, and help individuals connect with ongoing healthcare and supportive services.
What You’ll Do
Provide primary and basic medical care in community and street-based settings.
Perform medical assessments and monitor clients' physical health.
Diagnose and manage common acute and chronic medical conditions.
Provide basic wound care, vital signs, and other immediate medical services.
Prescribe and manage medications within your scope of practice.
Prescribe buprenorphine for individuals with opioid use disorder.
Build trusting relationships with individuals experiencing homelessness and substance use.
Connect clients with primary care, specialty care, behavioral health, substance use treatment, housing, safe havens, shelters, and other community resources.
Accompany established clients to medical appointments when needed to help them successfully connect with ongoing care.
Work collaboratively with Neighborhood Navigators and other members of the care team.
Complete clinical documentation and administrative responsibilities.
Provide care using a harm-reduction, trauma-informed, and person-centered approach.
