Job description
HarmonyCares is hiring a Nurse Practitioner to deliver home-based primary care to medically complex, home-limited adults. You’ll conduct in-home and telehealth visits, perform comprehensive assessments, manage chronic conditions, and prescribe medications. Working within an interdisciplinary team, you’ll coordinate care plans, support families and caregivers, and help reduce avoidable hospitalizations. This role is ideal for a compassionate NP who values autonomy, patient-centered care, and meaningful relationships in a mission-driven organization.
