About the Opportunity
A mission-driven, physician-owned organization is expanding its care model and seeking a Primary Care Nurse Practitioner to deliver personalized care to adult and geriatric patients in home and facility-based settings across the Greater Cleveland area.
This is not your typical clinic role. You'll enjoy full autonomy, a flexible schedule, and a collaborative support system that includes clinical leadership from a nationally recognized DNP. You'll provide general health maintenance and chronic disease management to patients already being served through a wound care program, adding value and continuity to their care journey.

What You'll Do
Deliver in-home and facility-based primary care to adult and geriatric patients
Diagnose and manage chronic and acute conditions with a holistic lens
Coordinate care with nurses, ADONs, DONs, and clinical teams across care settings
Maintain timely documentation and collaborate with leadership on care protocols
Educate patients and families on disease prevention, medication adherence, and long-term wellness strategies
Participate in quality initiatives and leverage tech-enabled tools for care efficiency
What You'll Need Active Nurse Practitioner license in Ohio (MSN or DNP)
National board certification (AANP or ANCC)
Active DEA license + BLS/CPR certification
Minimum 2 years of experience in adult/geriatric primary care or home-based care
Strong clinical decision-making skills and comfort working independently
Empathy, professionalism, and a team-oriented mindset
Reliable transportation and ability to travel locally within the Cleveland metro
