What you'll do as a Primary Care Nurse Practitioner:
Deliver comprehensive nursing care using advanced clinical knowledge and evidence-based practice
Conduct physical exams across diverse populations using a structured, developmental approach
Make differential diagnoses and identify both medical and non-medical contributors to health
Create individualized treatment plans that improve outcomes and support long-term wellness
Collaborate with patients and families to build meaningful, personalized health plans
Ensure accurate, timely documentation aligned with Meridian standards
Champion ethical, compliant care in accordance with all regulations
What you bring:
A Master's degree from an accredited Nurse Practitioner program
An active (or eligible) Indiana RN license in good standing
Nurse Practitioner certification (Family Practice, Psych/Behavioral Health, or Pediatrics)
Eligibility or active prescriptive authority/DEA licensure
Preferably 2+ years of related experience
Community health or mental health experience (a plus!)
A valid driver's license, reliable vehicle, and current auto insurance
