Job description
Oak Street Health seeks a compassionate Nurse Practitioner to provide high-quality, team-based primary care to adults on Medicare in underserved communities. In this role, you will perform comprehensive assessments, manage chronic conditions, and deliver preventative care focused on improving outcomes and reducing hospitalizations.
